Added missing mime.conf file. web server was not starting up without it.

I've cleaned up the PHP code so it fares much better with various PHP linters (phpmd, phpcs) and conforms better with PSR-2 coding standards. Generally it will behave much better in IDEs now.
Also fixed one bug with a misnamed method call.
