Add "phase_bank" to PCEAS so that symbols defined in relocatable code have the destination bank as well as the 16-bit .PHASE'd address. This means that it is now possible for symbols in a .PHASE'd area to have an undefined bank if there wasn't one in the .PHASE expression.
