Changes

Added the following end-points for managing Forms

• GET /api/v2/forms
• POST /api/v2/forms
• GET /api/v2/forms/{id}
• DELETE /api/v2/forms/{id}
• PATCH /api/v2/forms/{id}

• Test case added to verify the Create, Read, Update and Delete Forms.
