Restructured Serial Port Libraries and fixes to App Folders

This release restructures the source code so it can be built with the free-as-in-beer Visual Studio 2013 Express for Desktop.

Restructured the serial port library and added a compatible super-set library for the FTDI USB to Serial Converter Chip Family.

The App Folder concept was a little incoherent and partially implemented in earlier versions. This has been clarified, completed and fully documented.

There is now full documentation for the interface for Console Customisations.

Bug fixes, particularly Sub-menu handling.

Pre-built binaries for Windows XP and later, with user documentation.
