Added concatenation options to allow for developers to require fields and combined field outputs.
Updated the README to discuss the concatenation abilities.
I can't seem to get my code block to look like the other code blocks.

I added in a method for concatenation as I needed it.
We are using the Geoform for our Addressing Personnel. We need ALL of the Addressing fields entered, even the ones that are just other fields put together. But we KNOW that users will have issues, especially with redundant entries so this customization is meant to avoid that. I have tested this code out and am putting it live tomorrow on our internal-only site. Since the GeoForm I am using will allow the end user to CREATE addresses in our system I cannot share it. But I will try to get video of it tomorrow.
Let me know what else is needed.
