Have the CSV data export use column names that match what the import expects in OALM #6

It would make importing to OALM much easier if the column names in the downloaded CSV file matched the names OALM expects.

Also, need to insert a column named 'Unit Type' that contains the word "Troop" in each row, as OALM won't set a unit number without it.

Also need a State column for the addresses, if it's left out on the form, insert a dummy one with "MI"
