I personally don't like to dwell on an exercise for too long. If I simply don't get how to solve it within a reasonable amount of time, then I do some other exercises and return to it later. The only option as of now seems to be to get a new exercise, or mark the exercise as complete. (Or doing it outside of the website, which is a fair argument ofc)
My proposition would be to also add a third category. Pressing the fourth button would skip the current exercise but remember that it had been visited before. This exercise should have some extra indicator when you return to it again. Perhaps you could even have a button which returns one of the marked exercises instead of giving you a random new one. //: # (Include some extra documents or examples if you want!)
(The icon is ofc just an example, and it should probably be placed one step further to the left)
Some sort of indicator that you have attempted this exercise previously without being content with the result.

That sounds like a good idea. I think it works best if I also add a "randomize from to-do" button besides just the "add to to-do" button, like you suggested. I just need to ponder how to implement it without cluttering the UI too much (especially on mobile devices where space is already limited)
Besides that users might get confused as to what button to press, so a solution might be to add an "advanced options" toggle or something similar
