Creating a RFP template, example, issue type

Follow-up to #14 and #43

This is a top-level issue. A couple of tasks need to be done to complete RFP template:

• After PR review is merged, rename rfp-to-videographers.md to example-rfp-to-videographers.md.
• Incorporate the great feedback from Grace (create sub-issue, assign @patmatsu )
• Refine the document, until it is ready (in structure) for real-world use
• Copy the document to template-rfp-for-videographers.md and strip unnecessary sample text, add instructive comments (i.e. adding <!-- comment -->)
• Use the RFP template to create a new RFP Issue type
• Update the information model with an RFP section explaining template fields (Structuring campaign themes and campaigns #6)
• Copy the example RFP to 'Caffeine Injection' deliverable ('Reality Shock campaign) and update to latest status
