Create a solution for rendering health information.

Requires:

• selection of health district boundaries instead of / in addition of administrative boundaries
• health feature specific overlay stylesheet
• health feature specific index (see also Pluggable index types ocitysmap#68 )

Progress so far: a preliminary (very basic proof of concept) overlay style and alternative indexer exist, but are not rolled out to the public site yet.

The overlay style so far only renders health district borders ("boundary=health") and generic markers for "amenity=health_post".

The indexer also only indexes "amenity=health_post" POIs for now, and categorizes these by their "healthcare=..." value.
