We have a lot of custom fields and it would be far easier for us to specify a list of fields that we do want in the form rather than a list of fields that we don't.

interesting idea -- do you see this as being a separate box for specifying the desired fields? or perhaps a checkbox which toggles whether the defined field ids are exclusive or inclusive ?
I think a single input with a toggle would be clearest, possibly a dropdown
with the toggle options rather than a checkbox. Not overly fussed though :)
