Add banner showing environment and version to freezing-nq #4

If we add a banner that shows the environment and version, it will be easier to debug problems interactively when people report issues. We can also put this information into log messages while we run multiple environments.

Support for this is already in freezing-compose files.

We will need to add this to the local configuration examples too.

Having an environment variable is a practice I've used in so many places
and I want it here too.

The banner could show when doing a GET on the root document. It could even be text/plain.
