Describe the task
Accessibility audit of the new form that RST is proposing to use for authorizations.

Acceptance Criteria

Accessibility report saved in confluence: https://apps.nrs.gov.bc.ca/int/confluence/display/BCPRS/Accessibility+Audits
List of bugs/fixes needed is attached to report and saved in same location

Additional context

some of the basic website stuff we may not be able to resolve due to it being part of a larger system that we dont have control over (vFCBC) but we can change things within the form layout, format, flow, etc.
