Add a "report this problem" button to error notifications

Story

As a user, I would like an opportunity to report an issue as soon as it happens, so that I can remember the details of the issue and report it.

Acceptance Criteria

• GIVEN that an error notification is displayed
• WHEN the holder taps "report this problem" button
• THEN the contents of the error message will be sent to the Grafana dashboard
• AND the 'Report this problem' button should be disabled, and the button label should change to 'Reported' with a checkmark icon to the left of the button label.

Developer Notes

• TBD: Error messages should persist in Grafana for 'X' amount of time
