Are you using a modpack that has Mekanism Covers in it? I had the same problem. See link below.

I am having the same issue with ATM10. There seem to be some reliable causes for the shaders failing to load.

Reloading Shaders: 50/50 whether they fail to load.
Entering a new dimension e.g. the nether or any other modded dimension: The shaders remain active.
Exiting that same dimension: Shaders will always fail to load.

The issue seems to be limited to each shader pack, as I can work around this by loading a new shader pack or restarting my game.
Edit: The same issue occurs no matter what shader I am using, but loading a new one will fix it temporarily. Shaders that fail to load will remain broken until a restart.

The problem in ATM10 is Mekanism Covers. If you change disableAdvancedCoverRendering to true in the Mekanism Covers config, you can still use shaders. See link below.
